Description

We are seeking an LMHC or LCSW passionate about working with older adults to join our expanding community mental health team. This licensed professional will deliver direct mental health services, create individualized treatment plans, supervise and mentor Certified Peer Specialists, review peer documentation for quality and compliance, and collaborate with multidisciplinary partners to support seniors’ emotional health.

Requirements

Conduct in-home and community-based mental health assessments for seniors; Develop and update individualized treatment plans in collaboration with clients, peers, and other providers; Provide individual counseling, psychoeducation, and short-term mental health interventions tailored to older adults; Supervise Certified Peer Specialists; Coordinate care with medical providers, community resources, and family members
