Job Summary
We’re seeking a Behavioral Health Coach to support patients with practical, lifestyle-based strategies that complement psychiatric care. In this non-licensed coaching role, you will use the American College of Lifestyle Medicine framework to assess needs, co-create care plans, and provide ongoing support through brief sessions and check-ins. You’ll collaborate closely with our psychiatrists, psychiatric NPs, and care coordinators, offering guidance and scaffolding goals. This role contributes to our care management model and focuses on patient education, accountability, and behavior change—always within scope and in alignment with the care team’s plan.
Responsibilities
- Lifestyle assessment and planning
- Conduct structured lifestyle assessments using the American College of Lifestyle Medicine framework (e.g., nutrition, movement, sleep, stress, substance use risk reduction, social connection).
- Co-create personalized care plans and achievable SMART goals aligned with the psychiatrist/NP’s treatment plan.
- Coaching and patient engagement
- Provide brief, focused 1:1 coaching sessions (ongoing average ~30 minutes per patient per month; first month may be higher as needed).
- Offer mindfulness-based support (e.g., guided meditations, breathwork) to build stress-resilience and adherence.
- Use secure messaging and phone outreach between sessions for encouragement, accountability, and care coordination.
- Care collaboration and coordination
- Work in partnership with psychiatrists, NPs, and care coordinators; contribute to comprehensive care plans and follow-through.
- Facilitate referrals via the care coordinator when needed (e.g., therapy, community resources), and track follow-up.
- Documentation and care management
- Document goals, interventions, patient progress, and outreach in the EHR in a timely manner.
- Review existing assessments and outcome measures recorded by the care team; no additional measures administration required.
- Contribute to monthly care management minutes through live sessions, outreach, and documentation, per clinic protocols.
- Safety and scope
- Follow clinic risk screening and escalation protocols; training provided.
- Practice within a coaching scope (no diagnosis, psychotherapy, or medication recommendations).
Requirements
Qualifications
Required
- Bachelor’s degree in psychology, health sciences, lifestyle medicine, or a related field.
- National Board Certified Health & Wellness Coach (NBC-HWC) or an ICF coaching credential (Level 1/ACC, Level 2/PCC, or higher).
- Minimum 1 year of coaching experience (or 100+ client hours) in health, mental health, or related settings.
- Demonstrated skills in behavior change methods (e.g., motivational interviewing, goal-setting, habit formation).
- Comfort leading brief guided mindfulness and breathwork practices.
- Tech-savvy and comfortable working in an EHR and telehealth environment; strong written documentation skills.
- US-based, available Monday–Friday between 9:00 a.m. and 6:00 p.m. PST.
- Work location: either on-site in SF office or remote.
- Commitment to trauma-informed, culturally responsive, LGBTQIA+ affirming care and a collaborative team culture.
- HIPAA compliance (training can be provided upon onboarding)
Strongly preferred
- Formal MBSR/mindfulness training.
- Coaching skills training in CBT-informed strategies; CBT-I/sleep coaching; ADHD coaching.
- Experience collaborating with psychiatric or primary care teams.
